The Signal โ€” start here
Today's digest is dominated by two parallel themes: agent security hardening and Claude Code's remarkable velocity. On the security side, Anthropic landed three breaking behavior changes in Managed Agents (web_fetch now restricted to URLs already seen in-session, allowed_hosts governing web tools, and session creation failures for mismatched domain configs), published a usage policy update with real developer requirements for physical-action hardware, and launched the Cyber Mission's OSS Scanner. On the velocity side, Claude Code shipped three releases in under 24 hours (v2.1.293 yesterday, v2.1.294 and v2.1.295 today), with v2.1.295 being a ~100-item release bringing hooks onFailure: "block", an 8ร— increase in MCP tool description character limits, and a new mod notification API. These aren't separate stories โ€” they're the same story: the agent platform is hardening from "works in demos" to "safe to run in production."

Breaking Changes

โ—Breaking

Anthropic Managed Agents: three web-access security hardening changes

What changed
Three simultaneous breaking behavior changes to Managed Agents with limited networking, none of which were covered in yesterday's digest:

  1. web_fetch restricted to URLs already present in the session. The web_fetch tool now only fetches URLs that appeared in the session before the call โ€” in user message text, web_search results, or prior web_fetch results. URLs from Claude's own output, the system prompt, attached documents, or other tool outputs (bash, read, MCP) are not eligible. Calls for ineligible URLs return url_not_in_prior_context.

  1. allowed_hosts now governs the web_search and web_fetch tools. In limited networking, the same allowed_hosts list that gates network access now also filters web tool output: web_fetch calls to unlisted hosts return url_not_allowed; web_search omits results from unlisted hosts.

  1. Session creation now fails if allowed_domains contains entries not covered by allowed_hosts. With limited networking, creating or updating a session with a web tool's allowed_domains entry that isn't in allowed_hosts returns a 400 error. Note that docs.example.com is NOT within ["example.com"] โ€” each host must be listed exactly unless using a *. wildcard prefix.

TL;DR
Three Managed Agents web-access restrictions shipped Oct 7 โ€” web_fetch now only reaches URLs already seen in-session, allowed_hosts governs web tool output, and mismatched allowed_domains/allowed_hosts configs fail at session creation time.
Developer signal
Audit Managed Agent integrations in this order: (1) web_fetch calls: any agent that fetches URLs it generates itself (e.g., constructing a URL from tool output and fetching it) will now get url_not_in_prior_context errors โ€” send those URLs in a user message instead, or use a web_search step first to get the URL into context. (2) allowed_hosts vs allowed_domains: if your agent's web tools have allowed_domains entries, verify each is also in allowed_hosts or session creation will fail with a 400. Watch for exact-match vs wildcard semantics โ€” example.com does not cover docs.example.com. (3) Impact on agentic loops: agents that used web_fetch for data exfiltration defense bypasses (i.e., constructing URLs in-flight) are now stopped at the platform level โ€” this is the stated intent. Adjust agent prompts and tool configurations accordingly.

Affects you ifYou build with Managed Agents using limited networking; you have agents that call web_fetch on URLs they generate themselves; you have allowed_domains entries on web tools.EffortModerate โ€” requires auditing agent web_fetch call patterns, regenerating session configs that set allowed_domains, and updating agent prompts to pass fetchable URLs in user messages.

Model Releases

No new model releases today. Claude Haiku 5.5 launched October 7 (covered in yesterday's digest). Mistral Large 4 remains in API preview (covered October 7).

Claude Code v2.1.295 โ€” hooks `onFailure: "block"`, MCP descriptions 16K chars, gateway models list, mod notifications

What changed
A ~100-item release. Key changes by category:
TL;DR
Claude Code v2.1.295 adds onFailure: "block" for hooks (actions blocked when a hook fails, not silently passed), raises MCP tool descriptions to 16,384 chars, and adds a per-upstream models list for gateway routing โ€” update immediately if you use hooks for security controls or run long MCP tool descriptions.
Developer signal
Three changes demand attention before updating production Claude Code deployments: (1) onFailure: "block" changes hook semantics: previously, a hook that failed to start or timed out let the underlying action through. Now it blocks. If you have hooks that occasionally fail or time out (flaky HTTP endpoints, slow commands), those actions will now fail instead of proceeding. Audit hook reliability before deploying. (2) MCP tool descriptions now 16,384 chars: if you maintain MCP servers with rich tool descriptions that were previously silently truncated at 2,048 chars, update and re-test โ€” Claude now receives the full description, which may change behavior. (3) Gateway models list: if you self-host a gateway, add a models list to each upstream to control which models route there; without it, any model request goes to any upstream (existing behavior unchanged, but now opt-in granularity is available). Mod developers: $.ui.notify is the new notification primitive โ€” use it instead of custom toast workarounds.

Affects you ifYou use Claude Code hooks for security controls or automation; you maintain MCP servers with descriptions >2,048 chars; you operate a Claude apps gateway; you build Claude Code mods.EffortQuick (version update) โ€” audit hook onFailure semantics before deploying to production if hooks guard security-critical actions.

Research

arXiv (export.arxiv.org) and huggingface.co/papers egress-blocked this scan cycle. One candidate paper (AutoCompact on context management for long-horizon coding agents) was located via secondary abstract mirror but not read from its primary source โ€” excluded per non-negotiables. OpenAI Codex rust-v0.162.0 โ€” managed Git worktrees, live web for custom providers, ranked tool search, CRLF preservation

What changed
Managed Git worktrees for trusted local projects (create and list via new worktrees tools when the feature is enabled). Pinning tasks in the agent Command Center with p, grouped in a Pinned section. Live web access and remote compaction configuration for custom Responses-compatible model providers โ€” meaning any provider that implements the Responses API spec can now get live web access in Codex without being OpenAI. CRLF line endings preserved in apply_patch updates by default. Opt-in ranked tool search in Code Mode. Clickable URLs in approval prompts and banners. JavaScript streaming helpers. Linux sandbox fix for startup with multiple denied files. Signed PowerShell installer for Windows.
TL;DR
Codex rust-v0.162.0 ships managed Git worktrees for trusted projects, extends live web access to any Responses-compatible provider (not just OpenAI), and opts in to ranked tool search in Code Mode โ€” a meaningful release for self-hosting and multi-provider setups.
Developer signal
Two developer-facing changes worth acting on: (1) Live web for custom Responses-compatible providers: if you run Codex against a self-hosted or third-party provider implementing the Responses API, live web access is now configurable for your provider โ€” check the providers config for a web_access option. This matters for teams using Codex with Mistral, open-weight providers, or proxy layers. (2) Managed Git worktrees: if you use the worktrees feature flag, new CLI tools let Codex create and list worktrees from trusted local projects directly โ€” useful for parallelizing agent work across branches without manual setup. Note: rust-v0.163.0-alpha.1 released the same day; use v0.162.0 for stable operation.
Affects you ifYou run Codex against a custom Responses-compatible provider (non-OpenAI); you use Codex with the worktrees feature; you work on Windows (CRLF + signed installer).EffortQuick โ€” update to rust-v0.162.0.

Anthropic Cyber Mission โ€” OSS Scanner (free AI-powered vuln scanning for open-source projects)

What changed
Anthropic launched the Cyber Mission, an expansion of the earlier Project Glasswing. Two developer-relevant programs: (1) OSS Scanner โ€” opt-in, free periodic security scans of enrolled open-source projects using Anthropic's most capable models. Each report includes a PoC exploit, explanation, and suggested fix. Reports are model-generated without human review; Anthropic expects >90% true-positive rate. Projects without triage capacity can opt for human-verified disclosures instead. (2) Critical Infrastructure Defense Program (CIDP) โ€” frontier Claude access and on-site Anthropic engineers for security teams protecting operational technology (power grids, water systems), through founding partners including CrowdStrike, Palo Alto Networks, Dragos, and Nozomi Networks.
TL;DR
OSS Scanner is free AI-powered vulnerability scanning for open-source projects โ€” enroll at the link, get periodic reports with PoC + fix suggestion, Anthropic-backed without human review (>90% TP rate expected).
Developer signal
If you maintain an open-source project: enroll in OSS Scanner for free automated vulnerability reports from frontier Claude models. The key caveat is that reports are model-generated without human review โ€” triage time is on you. For high-traffic projects with limited maintainer bandwidth, Anthropic offers to route to human-verified disclosures instead. If you're on a security team working on critical infrastructure OT environments: the CIDP registration link is at the announcement URL. For OSS maintainers also eligible for Claude for Open Source: applications for free Claude Max subscriptions are open through the same program.
Affects you ifYou maintain open-source software and want automated security scanning; you work in security on critical infrastructure OT environments; you want free Claude Max access as an OSS maintainer.EffortQuick โ€” opt-in enrollment via the announcement page.

Anthropic 2026 Usage Policy update โ€” physical-action hardware requirements + deceptive activity section (effective November 12)

What changed
Usage Policy update effective November 12, 2026. Developer-relevant changes:

  1. Physical-action hardware requirement (new): Systems connecting Claude to autonomous physical hardware must now include a qualified operator stop capability and a defined safe-hold state that the hardware enters if Claude is disconnected. References the (upcoming) Model Hardware Standard.

  1. Deceptive activity section (new): Consolidated rule against fake accounts, influence operations, and fabricated content โ€” applies to commercial use, not just political campaigns.

  1. Surveillance/monitoring clarified: Non-consensual tracking prohibited. Consented tracking (fraud monitoring, parental monitoring), content moderation, journalism, and legal research remain permitted. Claude cannot decide who to investigate, arrest, or charge.

  1. Elections rules narrowed: Removed blanket ban on personalized vote and campaign targeting; narrowed to prohibiting deceiving voters or disrupting elections. Personalized targeting is now permitted if not deceptive or misusing personal data.

  1. Weapons software expanded: Prohibitions now cover weapons software, components, and actions like arming drones or autonomous vehicles.

  1. Supported Regions enforcement clarified: Covers users physically in unsupported regions, entities incorporated or headquartered there, and entities majority-owned from those regions.

TL;DR
Six usage policy changes effective November 12 โ€” the physical-action hardware requirement is new and applies to any Claude integration that controls autonomous physical systems; review before building or deploying in that space.
Developer signal
Review before November 12 if you build in any of these spaces: (1) Physical-action integrations: if your product connects Claude to hardware that takes physical actions (robots, drones, IoT actuators, lab instruments), add an operator-stop capability and a safe-hold state before November 12 โ€” this is now required, not recommended. (2) Surveillance/monitoring features: confirm that any tracking or monitoring in your product is consented by the subject; re-read the new non-consensual tracking prohibition against your use case. (3) Election-adjacent tools: personalized targeting for campaigns is no longer blanket-banned, but deception and unauthorized personal data use still are โ€” adjust permissions accordingly if you've been avoiding this use case. (4) Weapons-adjacent code: if you generate or assist with software in the weapons/defense space, re-read the expanded weapons software section.

Affects you ifYou build products integrating Claude with physical hardware; you build surveillance/monitoring features; you build election or political tools; you generate weapons-adjacent software.EffortModerate โ€” policy review across affected products; some may require architecture changes (adding operator-stop/safe-hold to hardware integrations).

mvschwarz/openrig โ€” manage persistent teams of Claude Code, Codex, and Pi agents as durable infrastructure

Developer signal
Install with npm install -g @openrig/cli. Run rig setup --dry-run in your repo to preview what it configures before touching any files. Start small: rig specs preview starter --kind rig shows you the default team YAML, then rig up starter --cwd . boots it. The fastest way to see the value is rig send dev-build@starter '<small task>' then rig queue list โ€” having a named, persistent agent address changes the interaction model meaningfully. Back up hook and trust settings files before rig setup (the README says this explicitly).

Affects you ifYou regularly run more than one Claude Code or Codex session in parallel; you want coding agent sessions that survive reboots with their context; you're building a multi-agent coding pipeline and want a management layer.
